Paying for Benefits While on Your Unpaid Leave/Layoff

Some benefits may be continued for a limited time, contingent of any required contributions or premiums for associates and/or dependents when an associate is on approved leave and is not receiving payments through payroll. The balance of owed associate contributions for continued coverage will automatically be billed to the associate each month following the plan administrator’s uniform procedures. Associates are required to make timely payments to prevent a beak in or termination of coverage. If the associate ceases to make any required contributions, coverage ends at the end of the period for which any required contributions are paid.
